This Lunar Module Checklist Page was flown and used on the Lunar Surface during the Apollo 17 mission to the Moon and has smudges of Lunar Dust. It also has hand written annotations made while inside the LM on the lunar surface

This page from the Apollo 17 LM Lunar Surface Checklist spent 3 days on Moon and was used by the Apollo 17 Crew as they prepared to leave the Valley of Taurus Littrow on their final day. It contains lunar dust and handrwritten notations made whilst on the lunar surface.

Side 7-9 shows LGC (Lunar Module Guidance Computer) and IMU (Inertial Measurement Unit) Power Up and a pre liftoff Eat Period, with hand written changes to the timings by Gene Cernan. These changes can be heard in the Apollo 17 transcripts:
"183:42:26 Fullerton: Okay, (on page) 7-9, LGC/IMU Power-up, change that time from 183:04 to 184:10. The eat period time is now 184:20.". Side 7-9 also has the most extensive lunar dust smudges on the right.

Side 7-10 deals with parking of the RR (Rendezvous Radar) Antenna and P57 Liftoff time, also with hand written annotations by Gene Cernan and Harrison Schmitt. These changes can be heard in the Apollo 17 transcripts:
"183:42:44 Fullerton: Turn the page (to 7-10), and the "183:59" above "Park Rendezvous Radar" is now 185:05.."
184:04:59 Fullerton: Okay. Page 7-10, your P57 lift-off time, upper right corner, is 188:01:43.85. Over..
Both sides have been hand signed by Cernan along with a stamp which reads,"Landed on the Moon aboard the Apollo 17 LM Challenger"
As this page was used after all 3 lunar EVAs, the Astronauts and the cabin of the Lunar Module would have been very much polluted with lunar dust, and as can be seen in the scans above, a lot of dust became smeared onto this page.
As it is not legal to buy Moon Rocks collected during the Apollo missions, items such as this are the only way to legally own any Apollo lunar material.
