This LM Cue Card measuring 6.5" x 7.5" was Flown to Earth Orbit on Apollo 9 - one of the most significant steps in all of the early Apollo flights as it was the Lunar Module's first test flight in space.
This particular Lunar Module cue card is entitled "LM Maneuver (P30)" The P30 maneuver was an Apollo Guidance Computer (AGC) program used to plan and execute major propulsive burns in both the Lunar Module (LM). As this mission marked the LM’s first crewed flight and served as its Earth-orbit shakeout test, the P30 cue card was an essential tool for mission success. It comes from McDivitt's collection, and includes a McDivitt hand signed LOA
Jim McDivitt has signed and inscribed the rear side as follows:
"FLOWN ON APOLLO 9 - Jim McDivitt - APOLLO 9 CDR"
The rear side is also fascinating in that the velcro tabs (all 3 of them) are still attached, so the card could be affixed to the LM control panel.
What makes this card particulary valuable is the extensive amount of hand written annotations made whilst in space. These numbers were read up to the LM from Houston, and were noted down in the grid provided on the card
This exchange is actually recorded in the mission audio which is transcribed below:
The numbers in yellow above, match the annotations on the front side of the cue card below

The rear side with McDivitt inscription "FLOWN ON APOLLO 9 - Jim McDivitt APOLLO 9 CDR" and 3 tabs of Velcro

Thanks

048:07:57 Schweickart (SPIDER): And, Houston, you got to give us the update at this time?
048:08:02 Roosa: Roger. I have the PAD ready to go, and can you take an uplink now?
048:08:09 Schweickart (SPIDER): Roger. The computer is yours; and ready to copy the docked DPS. This is Spider.
048:08:14 Roosa: Roger, Spider. Stand by one.
048:08:16 Scott (GUMDROP): Gumdrop is ready.
048:08:18 Roosa: Okay. Copied you, Gumdrop. And Spider, the uplink is on its way. I 049 41 3340, minus 00603, minus 17430, minus 00007 17440 all zips all zips, minus 00587, minus 17430, minus 00139. End of update. [Pause]
048:09:24 Schweickart (SPIDER): Roger, Houston. Spider reading back: 049 41 3340, minus 00603, minus 17430, minus 00007 17440 all zips all zips, minus 00587, minus 17430, minus 00139. [Pause]
048:09:57 Roosa: Roger. Very good. The readback was correct.
