This Schematic flew onboard the Apollo 13 Mission to the Moon in April 1970
Hand signed by Jim Lovell, Fred Haise and mission control EECOM, Sy Liebergot

This 2-fold schematic with tab labelled as "CB Function Pnl 8", is taken from the Apollo 13 CSM Systems Data Checklist, Part no. SKB32100082-340, Serial no. 1001 which was flown to Moon aboard the Apollo 13 Mission in April 1970.
The schematic shows functions powered thru circuit breakers on Panel 8 of the Command Module Cockpit, the rear side is blank.
When folded measures 8 x 10.5 inches, but when opened out fully it measures 18.5 x 10.5 inches and has been hand signed by Jim Lovell, Fred Haise and mission controller (EECOM) Sy Liebergot, who has added, "Looks like we have an instrumentation problem, Flight"
