This large 3-fold schematic entitled ,"SC 108 Effect of CB Opening" was flown to the Moon on Apollo 12 in Novemeber 1969 as part of the Apollo 12 CSM Systems Data Checklist. When folded measures 8x10 inches, but when opened out fully it measures 18.5 x 10 inches.
It shows some very interesting switches and circuit breaker settings that controlled the spacecrafts's power and guidance systems as well as environmental controls.
The schematic comes from the personal collection of Apollo 12 Command Module Pilot, Dick Gordon, who has also signed and inscribed the left hand side with, "Flown To The Moon on Apollo 12 - Richard Gordon CMP"
