Apollo 15 Flown to the Lunar Surface EVT Card

One of my favourite Apollo 15 pieces in my collection, this flown to lunar surface cue card, entitled, "Contingency EVT (2 OPS)" It details the steps needed if after lunar liftoff the LM was unable to dock to the CSM, or did dock but the tunnel was not able to be used, then the crew would need to perform an 'Extra Vehicular Transfer'. This would mean donning of suits with OPS unit for breathing, opening hatches on both spacecraft, then CDR and LMP would need to exit the LM and spacewalk over to the CM hatch.
